If you are unable to be at your assigned polling place on election day, you have the right to an absentee ballot 1. SUBMIT a request for an absentee ballot by Tuesday November 1, 2016 to your County Board of Elections 2. FILL OUT the ballot. Note that you have the right to assistance from a designated agent. 3. SUBMIT the completed ballot so that it is received by Friday, November 4, 2016. In case of emergency, fill out the Emergency Absentee Ballot by Friday, November 4

CONTACT your County Board of Elections and ask whether your location is handicap accessible. If it is not: 2. SUBMIT a request for an alternative ballot by Tuesday, November 1, 2016 to your County Board of Elections. **If you arrive at your polling place and cannot access it because of physical disability, DON'T PANIC. Fill out an Emergency Alternative Ballot by 8:00pm on the day of the election. You have the right to vote!
